Transfusion Reaction Investigation

Test description

A laboratory clerical check of all documentation relating to the pre-transfusion sample is performed. Pre and post- transfusion venipuncture samples are visually inspected and compared for the presence of hemolysis. Preliminary tests may include the determination of patient's ABO group, DAT and a screen for the detection of atypical antibodies. Additional testing is performed based on the patient’s clinical symptoms and/or laboratory findings.

Specimen and requisition requirements

Specimen(s):
One full (1) (preferably two (2)) 5-7 ml EDTA (lavender) tubes mixed thoroughly by gentle agitation. Within 14 days of collection.
Label specimen with the required minimum information: patient’s last name, first name, middle name, PHN or unique identifier and date of collection.

Complete Requisition (must include):

Patient’s last name, first name, middle name, PHN or unique identifier and date of birth
Facility name, complete address, phone and fax number
Phlebotomist ID information
Date of collection
